All claims and documents must be submitted within 90 days of the treatment or invoice date. There are several options for submitting your claim and veterinary records including via the MetLife portal, email, fax or mail.
Complete & Signed Claim Form
A complete Claim Form will help you get the fasted reimbursement for covered claims. Claim forms should be completed by the pet owner, and much of the necessary information and documentation should be included within the records and documents from your veterinarian.
Veterinary Medical Records (SOAP Notes) from Incident
Commonly referred to as SOAP notes, these can be digital records or handwritten from your veterinarian. If this is your first claim with Pin Paws Pet Care, underwritten by MetLife Pet Insurance, we also require the last 12-months of veterinary records. If you adopted your pet and this is your first claim, we require the adoption paperwork and any veterinary records you may have.
Itemized Invoice from Incident
These invoices may be digital or handwritten and are also provided by your veterinarian’s office. To process your claim and maximize your reimbursement for covered claims, we must have an itemized invoice to show the charges broken down and not just a total cost.

You have the option to submit your claim via your MyPets online portal, email, text, fax or mail. To file a claim, be sure to send the claim form, veterinary records, and itemized invoice to get your claim processed as quickly as possible. You must submit a claim within 90 days of treatment in order to be eligible for reimbursement.
MyPets Online Portal
Click “Submit a Claim” in the menu to upload digital files of your veterinary records, SOAP notes, chart notes, etc.) and itemized invoice. Select the pet the claim is for. If you’re submitting a claim for each pet, you’ll need to upload the applicable documents for each pet (even if the documentation has both pets listed). Select whether it is a “New” or “Existing” claim. Select “Existing” when you need to send additional documents for a submitted claim. Click “Add Files”. Select the files needed to process the claim, then click OK. Verify all the files and documentation needed are listed on the screen. Click “Submit” to send the documentation to MetLife Pet Insurance.

You can always submit your claim via email. When doing so, please be sure to include your policy number and pet name in the body of the email. Please be sure to properly attach all records via email—don’t copy and paste within the body.
